Method: Project recruits through clinics in male friendly environments as well as through primary care, as many men do not use primary care services. Waistwatchers course takes educational and empowering approach shown to work effectively with men. Longer term waist watchers league is used to maximise adherence to changed lifestyle/long-term health benefits. Lead Contact: 73-77 South Gardens St Giles Lincoln LN2 4NE Title: Travel Plans which incorporates: School Travel Plans (STPs) Business Travel Plans (BTPs) Aims: The main aim of Travel Plans is to reduce congestion by encouraging and promoting healthier alternatives such as walking, cycling and/or using Public Transport. Method: The strategy is based on identifying schools in an advantageous position to develop and Travel Plan. The first point of action was to contact schools with the greatest potential to achieve positive modal change. Following this initial approach, all public schools have now been written to outlining what a school travel plan is and how the schools can go about developing one. A similar letter is currently going out to special and independent schools. In addition, the County Council has trained 50 representatives from schools in the process of developing a travel plan. To ensure the most effective use if the current resources; four Travel Plan Advisers are working and working with the knowledge that these resources may not be available post April 2006, the advisors are currently working with the schools that show the greatest commitment and enthusiasm to developing travel plans, such as schools that attended the training course. Schools are offered the opportunity to receive direct support from the STPAs to develop their Travel Plan. In order to obtain this support, each signs a Service Level Agreement outlining the commitment required for the school to develop a Travel Plan. The County Council will then provide up to 5 visits to support the school to complete its Travel Plan. General sports and recreation programme are an integral part of the Youth Service curriculum with all provision offering the opportunity for supervised access to structured and recreational physical activity. Outdoor pursuits opportunities are offered through targeted work in 11 schools, Solutions4 provision in 3 locations - Birchwood, Holbeach and Louth. In addition through Youth Councils, Uproject for up to 120 targeted or referred school leavers. Operation bewitched and night orientation exercises. Residentials - all provision offers young people at least 1 residential experience per year. LEA, schools. Method: Project ACES was started in 1989 by Physical Education teacher Len Saunders of New Jersey, USA. Len wanted to get one school in every state to exercise at the same time on a specific day in May to motivate his physical education classes to exercise. Len began a letter writing campaign to schools throughout the United States to see if they wanted to join his massive exercise event. Everyone who received his letter thought it was a great idea, and of course, wanted to be part of it. The first year of ACES was a huge success with children from 1200 schools participating in all 50 states and the island of St Croix. Len just figured he would organise this programme in 1989 and that would be the end of it. Little did he know that ACES would eventually turn into a worldwide event. Within months of completing ACES 1989, Len started receiving mail for the 1990 ACES event, which at the time did not even exist! The rest is history! If offering either exercise provision or training. 5. CBC approached by School Nurses who identified a need to work with inactive/ overweight children at Shelthorpe School. Initial contact - PE Co-ordinator. An 8-week programme of lunchtime physical activity sessions (SAQ) are provided for identified pupils by the Community Sports Coach (CSC). To assist with the sustainability of the pilot project the lunchtime supervisors have attended TOP PLAY training. The CSC will support and mentor the lunchtime supervisors throughout the programme. The CSC will also mentor the older pupils to enable them to lead sessions once CSC leaves. Pedometers will also be distributed to classes during school hours to encourage pupils to increase steps per day. The school nurses, supported by local dieticians will provide a whole school approach towards healthy eating. If successful, will roll out to other local schools. 6. CBC lead project in partnership with the Loughborough Forum of Older People. Is an annual event, coordinated with relevant organisations working with older people. Over 35 organisations attend the event to exhibit on the day – to distribute advice and information to participants. 7. Participants are offered safe and effective exercise under the guidance of a qualified B.A.C.R exercise professional (Cardiac Rehabilitation Officer). Referrals are received via the University Hospitals of Leicester and selected G.P Practices. 8. In co-ordination with Loughborough College (Mission Active) to provide Council employees with pedometers. Loan period = 1 month. Employees handed information booklets, giving advice and information on how to increase steps a day, in line with recommendations (10,000 steps a day). Inter department competitions held. Bassetlaw PCT have organised 2 successful walking events through Clumber Park, the next event is June 12th and we expect to double the amount of walkers, to 240. Walks consist of a 1, 2, 3, 5, and 8 mile health walks, with the 1, 2 and 3 mile routes accessible for disabled/wheelchair people. The walks are promoted through local media, poster, flyers and PCT, Council and National Trust websites. Target: Women Geographic Area: North Derbyshire concentrating on specific areas where there is a higher level of deprivation. Setting: Community setting, e.g. local church halls and community centres. Partnership: Derbyshire Dales District Council, High Peak Borough Council, Chesterfield Borough Council, North East Derbyshire District Council. Chesterfield Walk This Way, Peak Park Leisure Walks, Chesterfield PCT, North Eastern Derbyshire PCT, High Peak And Derbyshire Dales PCT, Sure Start, Various community groups and exercise classes, New Bodies Gym, Various Community exercise classes, Derbyshire County Council Lifelong Method: Each programme runs for 20 weeks comprising of 2 hour sessions for groups of around 15 women. The sessions include an appropriate exercise component, which includes and enables women with mobility or health problems to exercise whilst seated in a chair, followed by a health education session. The women are supported and encouraged to make seemingly small changes to their lifestyle, which eventually lead to bigger changes and increases self-efficacy. During the course the women are encouraged to ‘buddy up’ and identify other activities they would like to try. Quite often, this includes the whole group attending the local leisure centre, if there is one, or going walking together. Towards the end of the course the women plan how they are going to maintain the changes they have made. Change For Life was initiated, designed and developed by North Derbyshire Health Promotion Service and now employs four part-time project workers and one programme manager. All the project workers are nationally qualified exercise to music teachers with additional vocational qualifications in nutrition, older people and exercise referral and are members of the National Register Of Exercise Professionals. They have also attended further training, run by the Health Promotion Service on obesity, behaviour change, group work skills and course content. Project workers liaise with Primary Care Teams, Community Development workers, Resident Associations, Sure Start and local people to publicise the forthcoming courses. All the courses are held in local village halls and community settings and are publicised through local networks. The publicity instructs all women to see their Practice Nurse first, who then forward details of each woman to us, including information on their health status and relevant medications and they are then allotted a place on the course. Once on the course motivation is assessed and guidance given to ensure that the goals set by the women are realistic and achievable. Behavioural change status is assessed and self-esteem questionnaires are completed at weeks 1, 10 and 20. Anecdotal evidence is also collected. The women have their blood pressure, resting pulse and weight measured by the Practice Nurse at weeks 1, 10 and 20. Further optional measuring is carried out by the Project Workers on the course and includes hip and waist measurements at weeks1, 10 and 20. Once the 20-week course has ended, a questionnaire is sent out six months later and a year after that, to ascertain whether longer-term changes have been made and maintained, namely that they have increased their levels of physical activity. Target: All Geographic Area: High Peak and Derbyshire Dales. Setting: Community setting, in local leisure centres and walking in the surrounding areas. Partnership: High Peak Borough Council, Derbyshire Dales District Council, University Of Derby, North Derbyshire Health Promotion Service, Participating GP practices in the High Peak Dales PCT area, New Boddies Gym Buxton Method: People are referred onto the scheme by their GP practice, physio or community mental health team. The participants receive specialist support from a exercise referral advisor. This involves 1 to 1 consultations and then a programme of activity specifically designed for their needs, lasting for 10 weeks. The 10-week scheme is free of charge with a further 10 weeks of subsidised rates. After that the participant will pay either normal fees or concessionary rates depending on their circumstances. The scheme is seen as ‘getting people going’ – the crucial bit is empowering them to feel able to continue being more active once the scheme ends. It encourages people to do other activities (not just leisure centre based), basically anything that they are interested in and that is appropriate for them, this may include home based activities such as walking or gardening. The most common findings are that people become more active, feel more confident and can do things they did not believe they could before. They begin to prioritise physical activity into their lives and greatly value the specialist physical activity support they receive. S4S offers volunteering & paid opportunities Insideout - Provide (outside) climbing and team building opportunities Compass training - Life coaching and assertiveness training Method: FAB supports residents to take the first step towards a sustainable healthy lifestyle, by providing intensive, tailor-made one to one support to identify and overcome barriers to physical activity. FAB works with people who would NEVER consider themselves ‘sporty’ and who have NEVER been in a leisure centre. It reaches people that mainstream sports services don’t. Mentors work with participants to identify goals, barriers and solutions. Together they take part in physical activity. Participants received about 12 sessions - some need more and there is no limit. FAB fits around participants - working early mornings, evenings and weekends. The project helps with the cost of buying equipment like swimming costumes and accommodates participant’s needs e.g. walking with them to facilities. Mentors motivate, encourage, support and inform participants. As their physical health improves participants become interested in health eating, smoking cessation. FAB offers training, support and referrals other organisations. Participants lose weight, gain confidence, reduce medication, increase their mental health, make new friends, stop smoking, change the way they eat, take up training opportunities, try new experience and achieve things they never thought possible. FAB is free to any resident of Braunstone, in one year FAB has worked with150 people (nearly 1% of Braunstone’s population). Having undertaken many hundreds of workshops, conferences, performances and artwork creations, the number of focal locations in the UK where Salamanda Tandem practice has been established has been growing. The fastest-growing centres are in Nottingham, Birmingham, Manchester and Leeds. In trying to adopt appropriate working methods, Salamanda Tandem makes a broad political commitment : it hopes, in all phases of its work, to witness to its convictions. It aims to confront in its procedures both the joys, and the difficulties and dilemmas, that people daily encounter. So the company is necessarily as concerned with the creative processes involved as it is by the outcomes: it sees no absolute distinction between the two – its work is always work in process. It tries to stress this by: i) considering the physical and psychological barriers that obstruct people’s creativity; ii) imagining fresh ways to communicate through movement; iii) viewing all that people are capable of, especially in gesture, as potentially expressive; iv) resisting simplistic or commercially-dictated notions both of what is beautiful and of what is ugly or ordinary. Partnership: South Kesteven District Council, North Kesteven District Council, Leisure Connections, Lincs South West PCT, West Lincs PCT, Age Concern Kesteven, Lincolnshire Social Services Method: Lincs South West PCT, West Lincs PCT, North Kesteven District Council, South Kesteven District Council and Lincolnshire County Council pooled budgets in 2004 to fund the training for 12 people to train as EXTEND teachers. An extensive recruitment programme took place with the assistance of the local press. 86 people expressed an initial interest and through a process of information packs, presentations and interviews, 12 people were selected for the training. Target: All Geographic Area: Broxtowe, Nottinghamshire. Setting: Out and about in the Borough of Broxtowe-walks take place in various locations. There are two different levels of walk-easy walks were initially led by leisure centre staff and intermediate walks by volunteers. Partnership: two leisure centres, health promotion specialist, volunteer organisations and volunteer walk leaders ( who are also steering group members), local school which has specialist sports college status, community development from Borough Council and Regeneration division of County Council Method: Used model of’ walking the way to health initiative’ (WHI). As no substantial funding was available, a partnership approach was used to take forward the idea. This acknowledged that many partners in the LSP (in which volunteers take a large role) had reasons to want to promote walking (for social, environmental, health reasons or for reasons of community safety, community involvement etc) The HDA review also found evidence that community based initiatives to promote walking can be effective. Setting: Leisure centre based and a sports college Partnership: Local Authorities, Heartlands PCT, Community/Cultural Leisure Partnerships, Sports College Method: Practices are invited to register for the scheme. Health professionals within the practice refer low to medium risk patients on to the scheme. The patient contacts the centre to book an induction, which is given by a fitness instructor with additional training in activity referral. The patient completes a medical questionnaire and receives a fitness check from the instructor. If they are not suitable they are referred back to the GP, otherwise they are accepted onto the scheme and a fitness programme is devised for them. The patient receives 12 weeks of supervised activity and is encouraged by the fitness instructor to look at how they might incorporate exercise into their life generally. They receive a fitness check at 5 weeks to check their progress and another check at 12 weeks where they also fill in a completion questionnaire. A report of their progress and plans for further exercise is sent to their GP. Community consultations as well as opportunities to feedback from activities are periodically undertaken to ensure we are still meeting those needs and wants and seeing how we can improve and/or develop each project to benefit of the individuals. The approaches taken have been formulated under project headings, each has many elements to its services. To assist with the sustainability of the pilot project the lunchtime supervisors have attended TOP PLAY training. The CSC will support and mentor the lunchtime supervisors throughout the programme. The CSC will also mentor the older pupils to enable them to lead sessions once CSC leaves. Pedometers will also be distributed to classes during school hours to encourage pupils to increase steps per day. The school nurses, supported by local dieticians will provide a whole school approach towards healthy eating. If successful, will roll out to other local schools.
